A patient's urine sample is first ionized by an electrospray ionization process, and the electrically charged particles are sorted out based on their mass-to-charge ratio. The separated ions can then be detected by a tandem mass spectrometer which detects the fragments produced because of collision-induced decomposition of precursor ions.

Biochemistry analyzers also play a critical role in various medical research projects such as studies related to hepatitis C virus infection. This device uses electric fields to sort DNA fragments by lengths and determine the number of hepatitis C virus particles in each blood sample. Moreover, they are used to detect other conditions such as phenylketonuria (PKU) which is a genetic disorder affecting amino acid metabolism. Other conditions diagnosed with the help of biochemistry analyzers include maple syrup urine disease, homocystinuria, and hyperlysinem among others. All these diseases result from the lack of specific enzymes needed for processing amino acids. In maple syrup urine disease, patients have a distinctive sweet odor in their urine because they cannot break down branched chain ketoacids. The condition is diagnosed by measuring these acids using tandem mass spectrometry.
